Overview

The Case Manager monitors participant progress with service offerings provided to participants initiated by Outreach Specialists & Community Liaisons. Overseeing Guardian Ad Litem enrollment and requirements adhering to agreements as set forth in federal code, state  guidelines, and municipal contracts: 

Description:
· Tracks and documents participants academic and program curriculum progress.

· Perform weekly assessments of participant areas of improvement to ensure successful completion of academic and judicial requirements.

· Conducts onsite visits to maintain an active presence in participant’s journey.

· Accurately document and report case file findings to Program Manager.

· Consult with parent and or legal guardian to develop appropriate plan of care and program course of action.

· Consult with Compliance Manager on codes, guidelines, and case contract requirements to ensure service offerings and/or program compliance. 

Requirements

Bachelors degree in Social Science, Public Policy, Associates in Early Childhood Development, Education and Management or relevant years of experience in lieu of degree. 

Previous case management and/or guardian ad litem experience is desired.

Overview

Guide participants through the self-sufficiency and sustainability journeys to ensure participant success. Provides participants with access to advertised information and unadvertised resources to prepare participants for success:

Description:

 · Assist participants with job skills identification, profile/resume assessment, skills as talent marketing, interview prep, track assigned program courses.

· Assist participants with goal setting, monitors milestone progress, provides guidance on navigating government processes, and assists with document preparation for license reinstatement. 

· Perform information quality checks to ensure all enrollment requests and data collected are being processed efficiently. 

· Review social media platforms for accuracy and integrity.

· Train Outreach Specialists on social media quality, content appropriateness, and target audience effectiveness. 

· Measure social media analytics performance and provide Program Manager with monthly social media analytics reports. 

Requirements

3 relevant years of experience or an Associates degree and 6 months of experience or Bachelors degree 0 years of experience in lieu of relevant experience.
